Process Overview

There are four primary phases in the lab processing of drug test results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample within a lab’s system. This entails confirming the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and finalizing any outstanding data entry absent from an electronic chain of custody system.

Screening conducts an initial assessment for drugs of abuse. Although Screening is a cost-efficient method to exclude drug use in the majority of samples, a positive screen must undergo confirmation to be permissible in court. Samples showing a presumptive positive in Screening necessitate further confirmation.

Should a sample yield a presumptive positive during Screening, additional hair is extracted from the main specimen and set for Extraction. This phase involves extracting drugs from hair at considerably lower concentrations than other methods (e.g., urine or oral fluid), thereby making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The entire lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standard accreditation.
