What it is, How it works

Hair analysis is recognized as an effective method for identifying drug and alcohol consumption. It archives a lengthy timeline of alcohol and other substances due to biomarkers embedding within the strands of growing hair. Positioned near the scalp, hair offers an approximate detection window of up to three months for substances. Its collection is straightforward, relatively hard to tamper with, and easy to transport.

A 1.5-inch segment of approximately 200 hair strands (comparable to the diameter of a #2 pencil) closest to the scalp can yield 100mg of hair, considered the optimal sample for analysis and verification. For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg sample is suggested. We suggest using a jeweler's scale for precise measurement. If head hair is absent, a similar volume of body hair may substitute. When referencing head hair, we specify scalp hair uniquely. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The core phases in laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a specimen into a lab's operations. This entails verifying proper sealing and shipping of the sample, generating a random LAN (Laboratory Accessioning Number), and finalizing extra data entry not supplied via an electronic chain of custody system.

Screening is a preliminary assessment for drugs of abuse. While it is cost-effective for ruling out drug use in the majority of samples, any positive screen must be confirmed for legal admissibility. Samples with a positive preliminary result in Screening necessitate a subsequent confirmation.

Should a sample exhibit a presumptive positive in the Screening phase, additional hair from the original sample is used during Extraction. In this stage, drugs are extracted from hair at significantly lower concentrations compared to methods like urine or oral fluid, which is why hair drug testing is notably challenging.

Confirmation of any positive screening result is executed through techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. All samples exhibiting presumptive positives undergo washing before confirmation if required. The entire lab process ranging from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and conforms to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection span: Hair testing can trace drug use for up to 90 days, as opposed to urine tests that have a briefer detection span.
  • Challenging to manipulate: It is notably challenging to manipulate a hair drug test, enhancing result accuracy.
  • Captures use patterns: It unveils a timeline of drug consumption across an extended duration, not just recent use.

Limitations:

  • Incapable of detecting recent consumption: Detection of drugs in hair takes about 5-7 days to manifest.
  • Expense: Hair drug analyses tend to be pricier than alternative testing methods.
  • Result variation: Elements such as hair color and individual hair growth diversities can sway drug metabolite concentrations in hair.

Note: Commonly termed as "hair follicle tests", the examination actually evaluates the hair strand, not the follicle beneath the scalp.
