Collecting a 1.5-inch length of roughly 200 hair strands (equivalent to a #2 pencil's width) closest to the scalp yields 100mg of hair, which is the optimal quantity for analysis and verification. In the case of EtG, additional tests, and/or more than 10-panel assessments, 150mg of hair is advisable. It is suggested to measure the hair sample using a jeweler’s scale. If head hair is not available, a similar quantity of body hair may be used. When mentioning head hair, it specifically pertains to scalp hair; body hair encompasses all other hair types, such as facial or axillary.

Process Overview

There are four key phases in the lab processing of a drug testing outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial introduction of a sample into a lab's workflow. This stage involves verifying the sample's integrity in terms of sealing and shipment, assigning a unique LAN (Laboratory Accessioning Number), and entering any supplementary information not included through an electronic custody chain method.

Screening involves conducting a preliminary assessment for potential drug abuse. While Screening serves as an economical option to exclude drug usage from most samples, any positive screens require confirmation for legal validity. Samples that screen potentially positive must undergo additional verification.

If a specimen denotes a probable positive during the Screening phase, extra hair is retrieved from the original sample for the Extraction process. Here, drugs are isolated from hair in much lower concentrations than those in other methods (such as urine or oral fluid testing), which is why hair drug testing is considered the most complex procedure.

Confirmation of any positive results from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All samples indicating presumptive positives undergo cleansing as required before Confirmation. The full laboratory process from Accessioning to Confirmation is evaluated in compliance with both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
