If scalp hair is not available, a similar quantity of body hair may be used. When 'head hair' is mentioned, it refers solely to scalp hair. 'Body hair' encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab system, including verifying proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any supplemental data entry not covered by an electronic custody system.

Screening involves a preliminary swift evaluation for drugs. While Screening is cost-efficient for ruling out drug use in most cases, a positive result requires confirmation for court validity. Samples that test presumptively positive during Screening need a subsequent confirmation.

Should a sample test presumptively positive during Screening, additional hair is taken from the original sample for Extraction. At this stage, drugs are extracted from hair at significantly lower concentrations than other means (e.g., urine or mouth fluid), which renders hair drug analysis the most challenging to execute.

The Confirmation of positive Screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Samples testing positive are washed as necessary before Confirmation. The full laboratory process, from Accessioning to Confirmation, is subject to review under the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.
