For EtG, supplementary tests, and/or panels exceeding 10, a sample of 150mg is advised. We suggest using a jeweler’s scale for accurate measurement. If scalp hair is unavailable, a comparable quantity of body hair can be collected. Here, head hair refers specifically to scalp hair, while body hair encompasses other types such as facial, axillary, etc.

Process Overview

Four primary stages are involved in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes its initial processing within the lab's system. This step verifies that the sample was sealed and shipped correctly, assigns it a unique LAN (Laboratory Accessioning Number), and completes additional data entry not recorded by an electronic chain of custody system.

Screening provides a preliminary assessment for substances of abuse. It is a cost-efficient method for ruling out drug intake in most cases; however, for a positive screening result to hold up in court, it must be substantiated through confirmation. Presumptive positives in Screening necessitate further validation.

Should a sample yield a presumptive positive during the Screening phase, additional hair from the original sample is prepared for the Extraction process. Drugs are extracted at significantly lower concentrations from hair than from other sources like urine or oral fluids, making hair drug testing a highly challenging methodology.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Before confirmation, any presumptive positive samples are cleansed as required. The whole laboratory process, from Accessioning to Confirmation, is examined under the CAP (College of American Pathologists) Hair designation and accordance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
