For EtG, additional testing, and tests exceeding 10 panels, 150mg of the sample is suggested. We advise measuring the sample with a jeweler’s scale. In the absence of scalp hair, an equivalent volume of body hair may be collected. When referring to head hair, we specifically mean scalp hair. Body hair pertains to other forms of hair (facial, axillary, etc.).

Process Overview

The primary phases in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the first stage of integrating a sample into a laboratory’s systems. This step includes confirming the sample's proper seal and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry lacking in the electronic chain of custody system.

Screening involves an initial rapid evaluation for substance abuse. Although Screening is an economical method to exclude drug use in most samples, a positive screen requires verification to be accepted in court. Samples initially testing positive in Screening require a follow-up confirmation.

In instances where the Screening stage yields a presumptive positive result, additional hair is extracted from the original sample and processed for Extraction. Here, drugs are isolated from hair at substantially lower concentrations than alternative methods (e.g., urine or oral fluid), making hair drug screening particularly challenging.

Any positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positive outcomes are cleansed before confirmation as necessary. The complete lab procedure, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair criteria and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
