A 1.5-inch section of approximately 200 hair strands (equivalent to a #2 pencil size) nearest to the scalp will yield 100mg of hair, which is the optimal amount for screening and confirmation processes. For EtG testing, additional panels, and/or tests beyond 10 panels, an amount of 150mg is advised. Using a jeweler’s scale is recommended for measuring the specimen's weight. If scalp hair isn’t available, a comparable amount of body hair can be taken. "Head hair" pertains specifically to scalp hair, whereas "body hair" includes all other types (such as facial or axillary hair).

Steps in the Process

The laboratory processing of a drug test result primarily involves these four stages: Accessioning, Screening, Extraction, and Confirmation.

The Accessioning stage pertains to the initial intake of a sample into a laboratory’s tracking system. This step ensures the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data input not included via an electronic chain of custody system.

Screening is the initial rapid check for the presence of drugs. While Screening is a budget-friendly approach to exclude drug usage from most samples, a positive screen requires confirmation to be valid in legal settings. Presumptive positives in Screening mandate subsequent confirmation.

Should a sample test presumptively positive during Screening, further hair is extracted from the initial batch and prepared for Extraction. In this phase, substances are removed from hair at a much lower concentration than with other methods (such as urine or oral fluids), making hair drug screening a particularly complex method to perform.

Confirmation of any positive outcomes in Screening is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive specimens are washed as necessary prior to confirmation. From Accessioning to Confirmation, the entire laboratory procedure is reviewed under the CAP (College of American Pathologists) Hair designation and aligns with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
