What it is, How it works

Hair testing has emerged as a highly effective method for identifying drug and alcohol consumption. Hair captures a prolonged history of alcohol and other drugs by embedding biomarkers within the fibers of the developing hair shaft. When taken near the scalp, hair can offer roughly a 3-month detection timeframe for alcohol and other drugs. Hair is easy to gather, relatively resistant to tampering, and uncomplicated to transport.

A sample measuring 1.5 inches with roughly 200 hair strands (about the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal quantity for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the specimen is advised. It is suggested to weigh the specimen using a jeweler's scale. Should scalp hair be unavailable, a similar quantity of body hair may be taken. In the context of head hair, we mean scalp hair solely. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four fundamental steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial phase of integrating a sample into a lab’s system. This phase ensures the sample was sealed and dispatched correctly, assigns a randomized LAN (Laboratory Accessioning Number), and inputs any supplementary data not furnished by an electronic chain of custody system.

Screening entails an initial rapid scan for drugs of abuse. While Screening offers a cost-efficient method to exclude drug use on most samples, a positive screen necessitates confirmation to be valid in court. Any samples that test presumptively positive during Screening require further confirmation.

If a sample is deemed presumptively positive during the Screening phase, additional hair is taken from the original specimen for Extraction. At this point, drugs are extracted from hair at a significantly lower concentration than in other methodologies (e.g., urine or oral fluid), rendering hair drug screening the most challenging methodology to undertake.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptively positive are cleaned before confirmation as required. The comprehensive laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
