What it is, How it works

A 1.5-inch section comprising about 200 hair strands (comparable to the diameter of a #2 pencil) nearest the scalp yields 100mg of hair, an optimal sample for both screening and confirmation. For EtG, supplementary tests, or tests exceeding 10 panels, 150mg of the specimen is advised. Utilizing a jeweler's scale for weighing the specimen is suggested. If scalp hair is not available, an equivalent amount of body hair can be used. The term "head hair" refers exclusively to scalp hair, whereas "body hair" encompasses all other hair types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial integration of a sample into a lab’s system. This process includes verifying proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data not provided by an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. Although Screening is cost-effective for dismissing drug use in most samples, a positive screening result requires confirmation for legal admissibility. Any samples presumed positive in Screening necessitate secondary confirmation.

If a sample is presumed positive during the Screening phase, additional hair is extracted from the initial sample for Extraction. Here, drugs are extracted from hair at substantially lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology.

Confirmation of any positive screening outcome occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation when necessary. The entire lab process, from Accessioning to Confirmation, is audited under the CAP (College of American Pathologists) Hair designation and conforms to ISO / IEC 17025 standards.
