For EtG, supplemental testing, and/or panels exceeding 10, 150mg of hair is advised. Measuring the sample on a jeweler’s scale is recommended. If head hair is unavailable, a comparable amount of body hair is acceptable. References to head hair specifically indicate scalp hair; body hair encompasses all other types (such as facial or axillary hair).

Process Overview

The laboratory processing of a drug test follows four main steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial entry of a sample into the lab's system. This step verifies the sample's proper sealing and shipment, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.

Screening is a preliminary examination for drugs of abuse. Though Screening is economical for excluding drug use in most samples, a positive result necessitates confirmation for admissibility in legal contexts. Any samples that yield a presumptively positive result in Screening require further confirmation.

Should a sample be deemed presumptively positive during Screening, additional hair is extracted from the original sample for the Extraction phase. During this stage, drugs are extracted from hair at significantly lower concentrations than in methods like urine or oral fluid testing, underscoring the complexity of hair drug screening.

Confirmation of any positive Screening result utilizes methods such as GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptively positive undergo washing before confirmation, if required. The entire process from Accessioning to Confirmation adheres to guidelines set by the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 certification standards.
