What it is, How it works

When procured close to the scalp, hair can present a detection timeframe of up to about 3 months for alcohol and substances. Hair is straightforward to collect, somewhat challenging to tamper with, and convenient to transport.

A 1.5-inch sample consisting of roughly 200 strands closest to the scalp will yield 100mg of hair, which is the ideal sample for evaluation and verification. For EtG, additional tests, and/or examinations exceeding 10 panels, 150mg of the hair is advisable. We suggest weighing the hair on a jeweler's scale. If scalp hair is inaccessible, an equivalent volume of body hair can be used. When mentioning head hair, we specifically refer to scalp hair. Body hair encompasses all other species of hair (facial, axillary, etc.).

Process Overview

The four pivotal phases in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into a lab’s framework. This involves ensuring the sample was sealed and sent accurately, attributing a random LAN (Laboratory Accessioning Number), and finalizing any extra data entry absent from an electronic chain of custody system.

Screening entails an initial rapid evaluation for drugs of abuse. Screening is a cost-efficient manner to dismiss drug use in the majority of samples; however, a positive result requires subsequent confirmation to be acknowledged in legal settings. Samples that appear positive in Screening require follow-up verification.

If a sample shows tentative positivity in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this stage, the drugs are extracted from hair at much lower concentrations than alternative methods (e.g., urine, oral fluid), which is why hair drug testing presents the most complexity.

Verification of any positive Screening result occurs through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with assumed positivity undergo washing before confirmation, as required. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.
