When cut close to the scalp, hair can reveal a use pattern for alcohol and drugs spanning up to 90 days. Collecting hair is straightforward, challenging to tamper with, and convenient for shipping.

A hair section measuring 1.5 inches, consisting of roughly 200 strands (similar to a #2 pencil in thickness) from near the scalp, yields about 100mg—ideal for screenings and confirmations. For EtG, extended panels, and/or tests over 10 panels, 150mg is advised. Using a jeweler’s scale for weighing is recommended. If scalp hair isn't available, a similar amount of body hair can be used. "Head hair" refers exclusively to scalp hair, whereas "body hair" covers all other types (like facial, axillary, etc.).

Process Overview

The essential phases in the lab processing of drug testing results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes integrating a sample into a laboratory system. This step ensures the sample is sealed and transported correctly, assigns it a unique LAN (Laboratory Accessioning Number), and completes additional data entry not covered by an electronic chain of custody system.

Screening provides an initial evaluation for drug presence. While cost-efficient for excluding drug use in most samples, a positive screening needs further validation for court acceptability. Samples that test positive initially must undergo secondary confirmation.

If a sample tests positive during Screening, more hair is extracted from the initial sample for the Extraction phase. Here, drug concentrations are removed from the hair at lower levels than other methods (e.g., urine or oral samples), making hair drug testing particularly challenging.

Positive results from Screening are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All positive samples are cleansed before confirmation if necessary. The entire procedure from Accessioning to Confirmation is assessed under CAP (College of American Pathologists) Hair designation criteria and accredited to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
