What it is, How it works

Hair analysis has emerged as an effective method for detecting substance and alcohol use. Hair offers a long-term record of alcohol and drug use by securing biomarkers within the fibers of the hair as it grows. When taken near the scalp, hair can reveal up to a 3-month timeframe for detecting alcohol and drugs. Hair is easy to gather, hard to tamper with, and straightforward to transport.

A sample of 1.5 inches consisting of around 200 hair strands (approximately the diameter of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the ideal quantity for both screening and verification. For EtG, add-ons, and/or more than 10-panel tests, it is advisable to have 150mg of the sample. We suggest weighing the sample with a jeweler’s scale. If scalp hair can't be collected, an equivalent amount of body hair can be used. Notably, head hair specifically implies scalp hair. Body hair encompasses other types (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test outcome consists of four major phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the primary handling of a sample in a laboratory’s framework. This phase ensures the sample was correctly sealed and forwarded, assigns a random LAN (Laboratory Accessioning Number), and fills in any missing data entry not covered by an electronic chain-of-custody protocol.

Screening serves as an initial fast assessment for substance abuse. As a cost-effective method to eliminate drug use in most samples, a positive result in Screening needs further validation to be court-admissible. Samples preliminarily positive in Screening must undergo secondary confirmation.

For presumptively positive outcomes in the Screening phase, additional hair is extracted from the primary sample and prepped for Extraction. Here, drugs are isolated from hair at much lower concentrations than other techniques (such as urine or oral fluid), making hair drug analysis the most challenging technique to perform.

Positive Screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positive samples are washed before Confirmation as necessary. The complete laboratory workflow from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection span: Hair drug tests can identify drug ingestion for up to 90 days, unlike urine tests that offer a shorter detection range.
  • Resistance to manipulation: It's extremely challenging to falsify a hair drug test, enhancing the reliability of the results.
  • Provides usage history: It uncovers a drug use pattern over time, rather than only recent use.

Limitations:

  • Inability to detect recent ingestion: Drugs become identifiable in hair approximately 5-7 days post-use.
  • Higher cost: Hair drug tests generally cost more than alternative drug testing methods.
  • Results variability: Elements like hair color and individual hair growth differences can impact the drug metabolite concentration in hair.

Note: Although commonly termed as "hair follicle tests", the test examines the hair strand rather than the follicle beneath the scalp.
