A sample measuring 1.5 inches, containing about 200 hair strands (roughly the diameter of a #2 pencil) and closest to the scalp, yields 100mg of hair, ideal for initial screening and confirmation. For EtG, add-ons, or tests beyond 10 panels, a sample of 150mg is advised. The specimen should be weighed using a jeweler's scale. If scalp hair is unavailable, an equivalent amount of body hair is acceptable. "Head hair" refers exclusively to scalp hair, whereas "body hair" includes all other types (such as facial and axillary).

Process Overview

The laboratory analysis of drug test outcomes involves four stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary setup of a sample into the lab's system, ensuring proper sealing and shipping, generating a random LAN (Laboratory Accessioning Number), and finalizing any outstanding data entries not handled by an electronic chain of custody framework.

Screening provides a preliminary assessment for substances of abuse. Although the Screening phase is cost-efficient for dismissing drug use in most cases, positive results necessitate confirmation for legal admissibility. Presumptive positives require a secondary verification.

For presumptive positives in Screening, additional hair is extracted from the primary sample for the Extraction phase, which isolates drugs from hair at much lower concentrations than other methods like urine or oral fluid testing, highlighting the complexity of hair drug screening.

Positive Screening results undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positives are cleansed before confirmation as required. The procedures from Accessioning through Confirmation comply with the standards of CAP (College of American Pathologists) for Hair certification and ISO/IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
