The main advantage is a longer drug detection window, reaching up to 90 days, performed in an observed setting, and is very hard to falsify. Yet, for identifying drug use within the past 5 days, urine tests remain the most precise.

What it is, How it works

Hair testing has emerged as a robust method to detect drug and alcohol usage. Hair captures a long-term record of alcohol and other substances by embedding biomarkers within the fibers of the hair strand as it grows. When hair is gathered near the scalp, it can offer up to approximately a 3-month period of detection for alcohol and drugs. Collecting hair is straightforward, relatively hard to tamper with, and convenient to transport.

A 1.5-inch segment of roughly 200 hair strands (approximating the girth of a #2 pencil) nearest to the scalp provides 100mg of hair, suitable for screening and confirmation tests. For EtG, additional tests, and/or tests above 10 panels, a 150mg sample is advised. It is recommended to weigh the sample using a jeweler's scale. If scalp hair is not available, an equivalent quantity of body hair may be collected. "Head hair" denotes only scalp hair. "Body hair" includes all other hair types (facial, axillary, etc.).

Process Overview

There are four primary stages in the lab processing of drug test results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample into a lab’s system, involving verification that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and finalizing any extra data entry not covered by an electronic chain of custody.

Screening is an initial quick evaluation for drugs of misuse. Though Screening is cost-effective for excluding drug use in most cases, a positive result requires confirmation to be upheld in court. Any samples deemed presumptively positive in Screening undergo secondary confirmation.

If a sample tests presumptively positive in Screening, more hair is drawn from the initial sample for Preparation for Extraction. During this phase, drugs are withdrawn from hair at a much lower concentration compared to other methods (ex. urine or oral fluid), thus making hair drug testing the most challenging to execute.

Confirmation of any positive screening is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation if necessary. The complete lab process from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO/IEC 17025 standards.

Advantages of hair drug testing:

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
