For EtG, additional tests, or panels exceeding 10, a 150mg specimen is advised. We suggest utilizing a jeweler’s scale to weigh the specimen. If scalp hair is not present, an equivalent quantity of body hair is permissible. For head hair, we mean only scalp hair. Body hair encompasses all other hair varieties (beard, underarm, etc.).

Process Overview

The principal stages in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of initially processing a sample into a lab’s system. It includes checking that the sample was sealed and dispatched correctly, generating a unique LAN (Laboratory Accessioning Number), and completing any further data entries omitted by an electronic chain of custody system.

Screening is an initial rapid analysis for abused drugs. Though Screening is an efficient way to rule out drug usage in most samples, a positive screen necessitates confirmation to be valid in legal scenarios. Any samples that are preliminarily positive in Screening need a subsequent confirmation.

If a sample is preliminarily positive during Screening, more hair is taken from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of a positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as necessary. The whole lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair specification and the accreditation to ISO / IEC 17025 standards.
