What it is, How it works

When gathered from near the scalp, hair can reveal evidence of alcohol and drugs use for a period of up to about three months. Hair is easy to gather, relatively hard to tamper with, and simple to transport.

By collecting a 1.5-inch section of roughly 200 strands of hair (similar in size to a #2 pencil) closest to the scalp, you obtain 100mg of hair, which is the optimal amount for both preliminary screening and detailed confirmation. For the EtG, additional tests, or over 10 panel tests, a sample of 150mg is advised. It’s recommended to measure the sample on a jeweler’s scale. If scalp hair is unavailable, collecting an equivalent amount of body hair is an option. References to head hair specifically mean scalp hair. Body hair includes other types like facial or axillary hair.

Process Overview

The laboratory handling of a drug test sample comprises four critical steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes initial processing in a lab's system. It includes checking that the sample is sealed and delivered correctly, issuing a random LAN (Laboratory Accessioning Number), and completing any required data entry absent from an electronic chain of custody system.

Screening entails a preliminary swift check for the presence of drugs. It serves as a cost-efficient method for dismissing drug use in most samples, though a positive screen needs further verification to be valid for legal proceedings. Samples that test positive during Screening must go through a secondary confirmation process.

When a sample tests positive initially in the Screening phase, additional hair is separated from the primary specimen and set up for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than other techniques (like urine or saliva), making hair drug testing a more challenging methodology.

Positive screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. Each presumptive positive sample is washed before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
