For EtG, additional analyses, and/or screenings beyond ten panels, collecting 150mg is advisable. The sample should ideally be weighed using a jeweler’s scale. Should scalp hair be inaccessible, a comparable amount of body hair can be utilized. The term head hair refers exclusively to scalp hair, while body hair encompasses all other types (e.g., facial, underarm).

Process Overview

The laboratory processing of drug test results involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the lab's system. This comprises checking that the sample was securely sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and adding any supplementary data not included in an electronic custody system.

Screening conducts a preliminary rapid scan for drug abuse. Screening serves as a budget-friendly method to exclude drug use in most cases; however, a positive screen mandates confirmation for court acceptability. Samples that are presumptively positive in Screening require additional confirmation.

If a sample tests presumptively positive during Screening, more hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are retrieved from hair at a significantly reduced concentration than other techniques (e.g., urine or saliva), making hair testing the most challenging method to execute.

Confirmation of positive screenings is achieved through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Every presumptive positive sample undergoes washing prior to confirmation as necessary. The entire laboratory protocol from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair standards and ISO/IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
