What it is, How it works

Hair analysis is acknowledged as an effective resource for identifying drug and alcohol consumption. Hair strands capture a long-term record of drug and alcohol intake by incorporating biomarkers within the growing fibers. When gathered near the scalp, hair offers a detection timeframe of up to around three months for substances like alcohol and other drugs. It is straightforward to collect, challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches with roughly 200 hair strands (equivalent to the width of a #2 pencil) closest to the scalp yields 100mg of hair, which is the ideal quantity for screening and validation. For EtG, additional analyses, and/or screenings beyond ten panels, collecting 150mg is advisable. The sample should ideally be weighed using a jeweler’s scale. Should scalp hair be inaccessible, a comparable amount of body hair can be utilized. The term head hair refers exclusively to scalp hair, while body hair encompasses all other types (e.g., facial, underarm).

Process Overview

The laboratory processing of drug test results involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the lab's system. This comprises checking that the sample was securely sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and adding any supplementary data not included in an electronic custody system.

Screening conducts a preliminary rapid scan for drug abuse. Screening serves as a budget-friendly method to exclude drug use in most cases; however, a positive screen mandates confirmation for court acceptability. Samples that are presumptively positive in Screening require additional confirmation.

If a sample tests presumptively positive during Screening, more hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are retrieved from hair at a significantly reduced concentration than other techniques (e.g., urine or saliva), making hair testing the most challenging method to execute.

Confirmation of positive screenings is achieved through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Every presumptive positive sample undergoes washing prior to confirmation as necessary. The entire laboratory protocol from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair standards and ISO/IEC 17025 accreditation.

Advantages of hair drug testing:

  • Extended detection period: Hair drug tests can identify drug use for up to 90 days, unlike urine tests which deliver a shorter span of detection.
  • Hard to manipulate: Circumventing a hair drug test is very challenging, enhancing result accuracy.
  • Historical insights: It offers a pattern of drug usage over time instead of only recent consumption.

Limitations:

  • Inability to detect recent consumption: A period of 5-7 days is required for drugs to show within hair strands.
  • Expense: Hair drug tests often incur higher costs than other drug testing procedures.
  • Result variations: Influences such as hair color and individual growth variances can impact the levels of drug metabolites in hair.

Note: Despite often being labeled as "hair follicle tests", the procedure examines the hair strand, not the follicle under the scalp.
