In the absence of head hair, an equivalent quantity of body hair is acceptable. References to head hair imply only scalp hair. Conversely, body hair includes all other types (facial, axillary, etc.).

Steps Involved

The main stages of laboratory operations in delivering a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of entering a sample into a lab's database. This step ensures that the sample was correctly sealed and dispatched, assigns an LAN (Laboratory Accessioning Number), and addresses any pending data input not supported by a digital chain of custody protocol.

Screening represents an initial rapid assessment for substance misuse. While it serves as a cost-effective measure to exclude drug presence in most samples, any positive screen mandates verification for court viability. Samples flagged tentatively positive during Screening need secondary confirmation.

Should a sample yield a presumptive positive during Screening, additional hair is extracted from the original sample for the Extraction phase. Here, drugs are isolated from hair at significantly reduced concentrations compared to other methods (e.g., urine or oral fluid), thus making hair drug evaluation the most challenging method to execute.

Positive results from a screen are verified using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples showing presumptive positivity are appropriately cleansed prior to confirmation. The complete lab workflow from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair criteria and conformity with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
