What it is, How it works

Hair offers an extended-term record of alcohol and other drugs by capturing biomarkers within the fibers of the developing hair strand. When retrieved near the scalp, hair can deliver up to about a 3-month detection timeframe for alcohol and various drugs. Hair is easy to gather, relatively hard to tamper with, and simple to transport.

A 1.5-inch sample consisting of about 200 hair strands (comparable to the size of a #2 pencil) closest to the scalp will provide 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, additional testing, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest measuring the specimen using a jeweler’s scale. If scalp hair cannot be obtained, a comparable quantity of body hair may be collected. When mentioning head hair, we exclusively mean scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial preparation of a sample within a laboratory’s system. This process includes confirming that the sample was sealed and delivered correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data input not available from an electronic chain of custody system.

Screening involves an initial fast assessment for drugs of abuse. Although Screening is a cost-efficient means of excluding drug usage from most samples, a positive screen has to be confirmed for it to be valid in court. All samples that are preliminarily positive in Screening require further confirmation.

If a sample is preliminarily positive in the Screening phase, additional hair is removed from the original specimen and prepared for an Extraction. In this stage, drugs are derived from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is the most complex method to execute.

Confirmation of any initial positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positive samples are cleansed before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
