What it is, How it works

Hair testing has emerged as a significant method for uncovering drug and alcohol consumption. Hair offers an extended chronological record of alcohol and other drugs by encapsulating biomarkers within the fibers of the advancing hair shaft. When obtained near the scalp, hair can provide about a 3-month detection period for both alcohol and other substances. Hair collection is straightforward, somewhat resistant to tampering, and easy to transport.

A 1.5-inch segment of approximately 200 strands of hair (roughly the diameter of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal sample for both screening and verification. For EtG, supplemental tests, and/or assessments exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the specimen using a jeweler’s scale. In cases where scalp hair is unavailable, a comparable amount of body hair may be collected. By head hair, we specifically mean scalp hair. Body hair encompasses all other types (beards, underarm, etc.).

Process Overview

The laboratory processing of a drug test result consists of four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial processing of a sample within a laboratory’s framework. This encompasses ensuring the sample was securely sealed and transported correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data entry not captured by an electronic chain of custody system.

Screening is an initial rapid assessment for drugs of abuse. Although Screening is a cost-efficient method for ruling out drug use on most samples, a positive result from a screen must be validated to be considered valid in court. Any samples that show presumptive positives in Screening necessitate a secondary validation.

If a sample shows a presumptive positive in the Screening phase, more hair is extracted from the primary sample for Extraction preparation. During this process, drugs are separated from hair at a much lower concentration than in other techniques (e.g., urine or oral fluid), making hair drug screening the most challenging technique to conduct.

Validation of any positive screen outcomes is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before validation as required. The entire lab procedure from Accessioning through Confirmation is subject to review under the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection timeline: Hair drug tests can disclose drug usage for up to 90 days, unlike urine tests, which offer a shorter detection timeframe.
  • Difficult to manipulate: It is considerably challenging to manipulate a hair drug test, thus rendering the findings more reliable.
  • Offers past usage data: It can illustrate a pattern of drug use over a time span, rather than just recent use.

Limitations:

  • Inability to detect recent usage: It requires about 5-7 days for drugs to become detectable in hair.
  • Cost: Generally, hair drug tests incur higher costs than other drug testing techniques.
  • Result variations: Factors like hair color and individual disparities in hair growth can influence the concentration of drug metabolites in the hair.

Note: Despite often being termed “hair follicle tests,” the test itself examines the hair strand and not the follicle beneath the scalp
