What it is, How it works

When collected near the scalp, hair can provide nearly a three-month detection period for alcohol and drugs. Hair collection is straightforward, quite challenging to tamper with, and simple to transport.

A 1.5-inch segment comprising roughly 200 hairs (approximately the size of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, additional tests, and/or assessments exceeding 10 panels, 150mg of the sample is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair is not available, a comparable quantity of body hair may be taken. When discussing head hair, we are referring solely to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The four primary steps in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a laboratory’s system. This process includes ensuring that the sample was sealed and shipped correctly, allocating a random LAN (Laboratory Accessioning Number), and executing any further data entry not provided by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. Although Screening is an economical approach to exclude drug use in the majority of samples, a positive screening result must be confirmed for courtroom admissibility. Any samples that appear positive in Screening necessitate an additional confirmation.

If a sample shows a presumptive positive in the Screening phase, additional hair is taken from the initial specimen for Extraction. At this point, drugs are separated from hair at a substantially lower concentration than in other methodologies (e.g., urine or oral fluid), which is why hair drug testing is the most intricate methodology to execute.

Confirmation of any positive result from screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation as required. The entire laboratory process from Accessioning through to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and the accreditation of ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
