In recent times, the approach to drug testing has often included a hair follicle test. Numerous employers, courts, and Substance Abuse professionals now demand a hair drug test instead of the conventional urine test. Hair follicle tests are employed by zero-tolerance drug use employers, courts, and those on probation due to their benefits. Hair testing is increasingly recognized as an effective method for detecting drug and alcohol consumption. Hair captures a comprehensive history of alcohol and drug usage by encapsulating biomarkers within the strands as they grow. When collected near the scalp, hair can offer up to a three-month detection period for alcohol and other substances. The process of collecting hair samples is straightforward, somewhat resistant to tampering, and convenient for shipping.
A 1.5-inch segment of about 200 hair strands (approximately the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal sample size for initial testing and confirmation. For EtG, additional analyses, and/or tests exceeding 10 panels, a sample of 150mg is suggested. It is advisable to quantify the specimen using a jeweler’s scale. In cases where scalp hair is unavailable, an equivalent volume of body hair may be used. When specifying head hair, we mean scalp hair exclusively. Body hair encompasses all other types such as facial and axillary hair.
Process Overview
The laboratory processing of a drug test outcome involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning refers to the preliminary processing of a specimen into a lab’s system. This process ensures that the sample is properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and finalizes any additional data entry that an electronic chain of custody system may not have provided.
Screening is a preliminary assessment for substances of abuse. While Screening is a cost-efficient way to negate drug usage in most samples, a positive result from this stage requires confirmation for legal acceptability. Thus, any samples with a presumptive positive outcome in Screening necessitate a further Confirmation step.
If a sample tests presumptively positive in the Screening phase, additional hair is taken from the original sample and prepped for Extraction. In this step, compounds are extracted from the hair at significantly lower concentrations than other methods such as urine or oral fluid, making hair drug testing one of the most complex methodologies to implement.
Confirmation of any positive result in Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test positive presumptively are cleaned before Confirmation as needed. The complete lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the standards for ISO / IEC 17025 accreditation.

Note: Despite often being called "hair follicle tests", the analysis is performed on the hair strand and not the follicle beneath the scalp.
