A hair sample of 1.5 inches comprising about 200 hairs (roughly the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal quantity for testing and validation. For EtG, extra tests, or panels exceeding 10, a specimen of 150mg is advised. Using a jeweler’s scale to weigh the sample is recommended. If scalp hair cannot be obtained, a comparable amount of body hair is acceptable. By "head hair," we mean only scalp hair. "Body hair" includes all other types (e.g., facial, axillary).

Process Summary

The principal phases in the lab processing of a drug test include Accessioning, Screening, Extraction, and Verification.

Accessioning involves integrating a sample into a lab’s system initially. This process ensures that the sample was properly sealed and dispatched, allocates a random LAN (Laboratory Accessioning Number), and completes any unfulfilled data entry from an electronic chain of custody system.

Screening consists of a preliminary examination for substances of abuse. Screening is an economical way to exclude drug use in most samples; however, a positive result requires confirmation to be admissible legally. Any samples with presumptive positives in Screening necessitate secondary verification.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original specimen for Extraction. In this phase, drugs are removed from hair at much lower concentrations than in other techniques (e.g., urine or oral fluid), making hair drug screening the most complex methodology to perform.

Verification of any positive screening outcome is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as needed. The complete lab process from Accessioning to Verification is evaluated under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
