What it is, How it works

Hair testing has become recognized as a potent means for identifying drug and alcohol consumption. Hair retains a prolonged history of alcohol and drugs by encapsulating biomarkers within the fibers of the developing hair strand. When extracted near the scalp, hair can reveal up to an estimated 3-month detection period for alcohol and other substances. Hair is effortless to gather, relatively hard to tamper with, and straightforward to transport.

A 1.5-inch portion of about 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, considered the optimal sample for analysis and confirmation. For EtG, supplementary tests, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest weighing the sample on a jeweler’s scale. Should scalp hair be unavailable, an equivalent amount of body hair may be gathered. When mentioning head hair, we denote scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four primary steps included in the lab handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a specimen into a lab’s system. This encompasses confirming that the sample was sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any additional data input not offered by an electronic chain of custody system.

Screening consists of a preliminary swift inspection for drugs of abuse. While Screening is an economical strategy to exclude drug usage for most samples, a positive screening result must be confirmed to be permissible in court. Any samples that are initially positive in Screening require an additional confirmation.

If a sample is initially positive in the Screening phase, more hair is taken from the initial sample and set for Extraction. In this phase, drugs are withdrawn from hair at a much lower concentration compared to other methodologies (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is administered via GC/MS, GC/MS/MS or LC/MS/MS. All initial positive samples are cleansed prior to confirmation as needed. The entire lab procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair classification and the certification to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
