What it is, How it works

Hair testing is now regarded as a robust tool for identifying drug and alcohol consumption. It offers an extended history by embedding biomarkers within the fibers of the hair as it grows. When gathered near the scalp, hair provides up to around a 3-month detection timeframe for alcohol and drugs. It's easy to gather hair samples, challenging to tamper with, and simple to mail.

A 1.5-inch strand, roughly 200 hairs (similar to the diameter of a #2 pencil) closest to the scalp will yield 100mg, which is perfect for screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, 150mg is preferred. We advise using a jeweler’s scale to weigh the sample. If scalp hair is nonexistent, the equivalent amount of body hair can be used. When mentioning head hair, we strictly refer to scalp hair. Body hair encompasses other types like facial or axillary hair.

Process Overview

The primary steps in processing a drug test in the laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is where a sample is first logged into the laboratory system. This step verifies sealed and correctly shipped samples, assigns a LAN (Laboratory Accessioning Number), and completes any manual data input that the electronic chain of custody does not cover.

Screening acts as an initial quick drug abuse check. Although it’s an economical method to exclude drug use in most samples, a positive screen must be validated for legal admissibility. Presumptively positive samples in Screening need further confirmation.

When a sample tests positive in the Screening phase, more hair is taken from the original portion and set for Extraction. During Extraction, drugs are extracted from hair in much lower concentrations compared to other methods like urine or oral fluid, making hair testing the most challenging methodology.

Often via GC/MS, GC/MS/MS, or LC/MS/MS, Confirmation of positive screenings occurs. Samples that tested presumptively positive undergo washing before confirmation, as required. Every step, from Accessioning to Confirmation, adheres to both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extensive detection period: Hair drug tests identify drug use up to 90 days back, unlike urine tests with shorter detection.
  • Difficult to deceive: Hair tests are notably hard to manipulate, ensuring more reliable results.
  • Tracks usage patterns: It reveals drug use patterns over time, not just recent consumption.

Limitations:

  • Unable to detect immediate use: Drugs become detectable in hair after about 5-7 days.
  • Expense: Hair drug tests typically cost more than alternative methods.
  • Result variability: Hair color and individual growth differences can influence drug metabolite concentrations in hair.

Note: The term "hair follicle tests" is misleading; the analysis focuses on the hair strand, not the follicle under the scalp.
