A sample measuring 1.5 inches, containing around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp, will provide 100mg of hair, which is the optimal sample size for both screening and confirmation. For EtG, optional tests, and/or panels exceeding 10, 150mg of the sample is suggested. We advise measuring the sample on a jeweler’s scale. If scalp hair isn't accessible, an equivalent quantity of body hair can be obtained. In reference to head hair, we mean scalp hair specifically. Process Overview
The lab procedure for processing a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning is the initial stage where a sample is integrated into a laboratory’s system. This step involves checking that the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic chain of custody system.
Screening is the first quick evaluation for substances of abuse. This method is cost-efficient in excluding drug use in most samples, though any positive finding necessitates confirmation to be valid legally. Samples that show a presumptive positive in Screening must undergo secondary confirmation.
If a sample tests presumptively positive during Screening, additional hair is extracted from the original specimen for Extraction. At this phase, substances are extracted from hair at a lower concentration than in other methodologies (e.g., urine or oral fluid), rendering hair drug analysis the most complex methodology to execute.
Confirmation of any positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if needed. The full lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.
The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
