What it is, How it works

Hair tests are increasingly recognized as an effective method for detecting drug and alcohol consumption. Hair allows for the long-term retention of alcohol and drug markers within the growing hair strand's fibers. When obtained near the scalp area, hair offers nearly a 3-month detection period for alcohol and other drugs. The collection of hair is straightforward, it's not easily tampered with, and transporting it is convenient.

A sample measuring 1.5 inches comprising around 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp provides about 100mg of hair, which is the optimal quantity for screening and confirmation. For EtG, additional analyses, and/or tests exceeding 10 panels, a 150mg specimen is advisable. Using a jeweler’s scale is suggested for weighing the specimen. In the absence of scalp hair, a comparable quantity of body hair can be utilized. When discussing head hair, it refers exclusively to scalp hair. Body hair covers all other hair types (like facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four critical steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes initial processing within a laboratory's system. This phase ensures the sample's sealing and shipping are correctly handled, assigns a random LAN (Laboratory Accessioning Number), and finishes any supplementary data input not accounted for by an electronic chain of custody system.

Screening serves as an initial, quick assessment for potential drug misuse. While Screening is a budget-friendly approach to eliminate drug usage for most samples, legally valid results require confirmation of positive screens. Any samples deemed presumptively positive in Screening necessitate a follow-up confirmation.

Should a sample register presumptively positive during the Screening phase, additional hair is collected from the original sample and readied for Extraction. At this stage, drug substances are removed from hair at significantly lower concentrations than alternative techniques (e.g., urine or oral fluid), underscoring why hair drug screening is the most challenging technique to execute.

Confirmation of any positive screening findings is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as required. The entire lab process from Accessioning through Confirmation is assessed under the CAP (College of American Pathologists) Hair designation as well as ISO / IEC 17025 standards certification.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
