What it is, How it works

When clipped near the scalp, hair can reveal a detection timeframe of up to three months for alcohol and other drugs. A sample measuring 1.5 inches, approximately 200 hair strands (comparable to the girth of a #2 pencil), nearest to the scalp will produce 100mg, which is optimal for screening and verification. For EtG, additional tests, and/or panels exceeding 10, it's suggested to procure 150mg. It's advisable to use a jeweler’s scale to weigh the sample. If scalp hair is not available, an equivalent quantity of body hair may be gathered. When mentioning head hair, it pertains solely to scalp hair. Body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug test involves four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary integration of a sample into a lab's infrastructure. This entails ensuring the sample's seal and shipment integrity, allocating a random LAN (Laboratory Accessioning Number), and completing any outstanding data input absent in an electronic chain of custody setup.

Screening serves as an initial rapid analysis for substance misuse. Although Screening is a cost-efficient approach to negate drug use in most samples, a positive need verification to be court-admissible. Any samples indicating a presumptive positive in Screening mandate a subsequent confirmation.

If Screening implies a presumptive positive, additional hair is sourced from the original sample for Extraction. At this stage, drugs are drawn from hair at a much diluted concentration than other methods (e.g., urine or oral fluid), hence hair drug analysis is considered the most challenging methodology.

Confirmation of any positive screen is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample undergoes a cleansing before confirmation, if necessary. The entire laboratory operation from Accessioning to Confirmation is inspected under both CAP (College of American Pathologists) Hair recognition and accreditation to ISO / IEC 17025 criterions.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
