When collected near the scalp, hair can reveal alcohol and other drug use for up to approximately three months. Hair is easy to collect, relatively tough to tamper with, and simple to transport.

A 1.5-inch sample consisting of about 200 hair strands (roughly the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is optimal for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, it is advisable to collect 150mg of the specimen. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair is unavailable, an equivalent amount of body hair may be used. When discussing head hair, we refer exclusively to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four key phases in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is processed into a laboratory’s system. This involves verifying the proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not supported by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. Although Screening is a cost-efficient method to dismiss drug usage in most samples, a positive screening necessitates confirmation to be valid in a legal setting. Any samples presumptively positive in Screening require secondary confirmation.

Should a sample test presumptively positive during Screening, additional hair is extracted from the initial specimen and prepared for Extraction. This step involves removing drugs from hair at a much lower concentration than other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most challenging methodology to execute.

Confirmation of a positive screening result is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
