What it is, How it works

The use of hair testing has emerged as a potent method for identifying drug and alcohol consumption. Hair acts as a repository for a prolonged history of substance use by embedding biomarkers within the fibers of the developing hair strand. When acquired near the scalp, hair can reveal up to roughly a 3-month duration for detecting alcohol and other drugs. Hair is easy to gather, moderately challenging to tamper with, and straightforward to transport.

A sample measuring 1.5 inches with about 200 hair strands (comparable to a #2 pencil) next to the scalp will yield 100mg of hair, the preferred sample for examination and validation. For EtG, additional add-ons, and/or evaluations beyond 10 panels, a 150mg specimen is advised. We suggest weighing the specimen on a jeweler's scale. If scalp hair is not obtainable, an equivalent portion of body hair may be obtained. When mentioning head hair, we specifically refer to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The primary steps involved in laboratory processing for a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial handling of a sample into a lab's database. This step verifies the proper sealing and shipping of the sample, allocates a unique LAN (Laboratory Accessioning Number), and completes additional data entry not furnished by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for substances of abuse. While Screening is a cost-efficient means to exclude drug usage in the majority of cases, a positive result must be confirmed to be admissible in court. Any samples showing presumptive positivity during Screening require further confirmation.

In case a sample displays presumptive positivity during Screening, additional hair is taken from the original specimen and prepared for Extraction. This phase involves extracting drugs from hair at significantly lower concentrations than other methodologies (e.g., urine or oral fluid), making hair drug testing the most challenging methodology.

Confirmation of positive Screening results is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples with presumptive positivity are washed before Confirmation as deemed necessary. The entire lab process from Accessioning to Confirmation is audited under the CAP (College of American Pathologists) Hair classification and in accordance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
