For EtG, supplementary tests, and/or assessments exceeding 10 panels, it is advisable to have 150mg of the sample. We suggest using a jeweler’s scale for weighing. If scalp hair isn't available, a comparable quantity of body hair can be collected. 'Head hair' specifically refers to scalp hair, whereas 'body hair' pertains to all other types (facial, axillary, etc.).

Process Overview

The laboratory process for determining a drug test result includes four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a lab’s system. It involves confirming that the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and inputting any supplementary data not provided by an electronic chain of custody system.

Screening offers a preliminary assessment for drugs of abuse. While it serves as a budget-friendly method to eliminate drug usage in most cases, a positive finding in Screening demands verification for legal acceptance. Any samples that are initially positive require further confirmation.

If a specimen shows a positive indication during Screening, additional hair is extracted from the initial sample for Extraction. In this phase, drugs are isolated from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), which makes hair drug analysis the most challenging to conduct.

Confirmation of positive results from screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All pre-positive samples undergo a wash before confirmation as needed. The whole lab procedure from Accessioning to Confirmation is reviewed under the supervision of the CAP (College of American Pathologists) Hair accreditation and is compliant with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
