What it is, How it works

A 1.5-inch section of roughly 200 hair strands (about the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is the ideal quantity for both screening and confirmation. For EtG tests, additional tests, or tests that exceed 10 panels, a specimen size of 150mg is advised. We suggest measuring the specimen with a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair can be taken. When we mention head hair, we exclusively mean scalp hair, while body hair encompasses all other types (such as facial or axillary).

Process Overview

The laboratory processing of a drug test result comprises four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is processed into a lab’s system. This step involves checking the sample’s seal and shipment, generating a random LAN (Laboratory Accessioning Number), and completing any additional data entry that is not handled by an electronic chain of custody system.

Screening is a preliminary assessment to check for drugs of abuse. Whilst Screening offers a cost-effective approach to rule out drug consumption in most samples, a positive screening requires confirmation to be valid in a legal context. Samples that have presumptively positive results in Screening need secondary verification.

If a sample shows a presumptively positive result during Screening, additional hair is taken from the initial sample for Extraction. At this stage, drugs are extracted from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), which makes hair drug testing highly complex.

Confirmation of any positive screening result is conducted using techniques like GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positive findings are washed prior to confirmation if necessary. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair guidelines and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
