What it is, How it works

Hair testing has gained recognition as an effective method for detecting drug and alcohol usage. Within hair, long-term traces of alcohol and drugs are captured as biomarkers embed within the hair shaft during its growth. When hair is gathered close to the head, it can offer a detection period of approximately up to three months for alcohol and drug substances. Hair collection is straightforward, not easily tampered with, and convenient for transportation.

A 1.5-inch segment comprising about 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp will yield around 100mg, the ideal quantity for both screening and confirmation. For EtG add-ons and/or tests beyond 10 panels, a 150mg sample is advisable. It is recommended to use a jeweler’s scale for measuring the sample. Should scalp hair be inaccessible, a similar quantity of body hair can be used. When speaking of head hair, it specifically implies scalp hair, whereas body hair denotes all other types like facial, axillary, etc.

Process Overview

Lab processing of a drug test result involves four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the entry of a sample into a lab's workflow. This step ensures that the sample is sealed and transported correctly, allocates a LAN (Laboratory Accessioning Number), and completes any supplemental data entry that the electronic chain of custody might lack.

Screening serves as a preliminary check for drug presence. Although Screening is cost-effective for eliminating drug usage in most samples, a positive result requires confirmation for legal admissibility. All samples that test positive in Screening must undergo further confirmation.

If a sample appears potentially positive during Screening, additional hair is retrieved from the initial sample for the Extraction process. In this phase, drugs are extracted from the hair at notably lower concentrations than other approaches like urine or oral fluid, presenting a greater challenge in hair drug testing methodologies.

Confirmation of a positive screening result employs GC/MS, GC/MS/MS, or LC/MS/MS methods. Presumptive positive samples are cleaned before confirmation as needed. The complete lab process from Accessioning through Confirmation is examined under both CAP (College of American Pathologists) Hair criteria and conforms to ISO / IEC 17025 standards.
