What it is, How it works

Hair analysis has emerged as a robust mechanism for identifying drug and alcohol consumption. By capturing biomarkers within the strands, hair offers an extended timeline of substance use. When samples are cut close to the scalp, they provide a detection period for alcohol and other substances of up to roughly 90 days. Hair collection is straightforward, tampering is relatively difficult, and it is convenient for transport.

A sample measuring 1.5 inches in length, consisting of around 200 hairs (comparable to a #2 pencil), nearest to the scalp yields 100mg of hair, which is ideal for both screening and confirmation. For EtG, additional tests, and/or those beyond 10 panels, a 150mg sample is advised. Utilizing a jeweler's scale for weighing is suggested. If scalp hair is unavailable, an equivalent quantity of body hair is acceptable. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample undergoes initial processing within a lab's system. This step includes confirming the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data entry not covered by an electronic chain of custody.

Screening is a preliminary examination for drugs of abuse. Although Screening offers a cost-efficient way to dismiss drug use in most cases, a positive outcome necessitates confirmation for legal admissibility. Samples showing presumptive positivity in Screening will require further confirmation.

If a sample returns a presumptive positive during the Screening phase, more hair is sourced from the original batch for Extraction. At this stage, drugs are isolated from the hair at much smaller concentrations than other methods (like urine or oral fluid), making hair drug testing the most challenging methodologically.

Positive Screening results require Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to Confirmation, all samples presumed positive undergo washing. The comprehensive laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standard accreditation.

Advantages of hair drug testing:

  • Extended detection period: Hair tests can uncover drug use up to 90 days, unlike urine tests, which have a shorter detection timeframe.
  • Harder to manipulate: It's challenging to deceive a hair drug test, enhancing result accuracy.
  • Offers historical insights: It reveals drug use trends over time, not just recent consumption.

Limitations:

  • Inability to spot recent use: Drugs become detectable in hair only after about 5-7 days.
  • Expense: Hair drug testing tends to be costlier than other methods.
  • Result variability: Hair color and personal growth rate differences can influence drug metabolite concentration.

Note: Though commonly termed "hair follicle tests," the actual test examines the hair strand, not the follicle beneath the scalp
