A 1.5-inch section consisting of around 200 hair strands (approximately the size of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal sample size for analysis and verification. For EtG, additional components, and/or tests exceeding 10 panels, 150mg of the sample is advisable. It's suggested to weigh the sample on a jeweler’s scale. If scalp hair isn't accessible, a similar amount of body hair may be gathered. By head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result comprises four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the introductory step of processing a sample into a laboratory’s framework. This step includes checking that the sample is properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any other data input not covered by an electronic chain of custody system.

Screening serves as an initial rapid assessment for drugs of abuse. While Screening is a cost-effective approach to dismiss drug usage in most samples, a positive screening result necessitates confirmation for it to be viable in a legal context. Presumptively positive samples in Screening require further confirmation.

If during the Screening stage a sample shows a presumptive positive, more hair is drawn from the initial sample and readied for Extraction. At this phase, drugs are extracted from hair at a much lower concentration compared to other techniques (e.g., urine or oral fluid), illustrating the complexity of hair drug screening methodologies.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positive results undergo washing prior to confirmation as needed. The full laboratory process, from Accessioning to Confirmation, is conducted under the scrutiny of both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
