For EtG, supplementary tests, or panels exceeding 10, 150mg of the sample is suggested. It's advisable to measure the sample with a jeweler's scale. If scalp hair isn't available, the equivalent amount of body hair may be used. "Head hair" refers strictly to scalp hair. "Body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The primary steps in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails processing a sample into the lab's system. This process includes confirming the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing data entry not supplied by an electronic chain of custody system.

Screening involves an initial quick assessment for drug presence. Although Screening is a cost-effective approach to exclude drug usage in most samples, a positive result mandates confirmation to be valid in legal contexts. Samples that are presumptively positive in Screening require a follow-up confirmation.

If a sample is presumptively positive in the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are removed from hair at lower concentrations compared to other methods (such as urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any presumptive positive results is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation. The complete laboratory process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
