What it is, How it works

It offers a prolonged history of substance use by capturing biomarkers within the fibers of the growing hair shaft. When hair samples are taken near the scalp, they can provide roughly a 3-month detection time frame for alcohol and drugs. The collection of hair is straightforward, somewhat challenging to tamper with, and easy to send.

A sample of 1.5 inches comprising about 200 hair strands (roughly similar to a #2 pencil) taken closest to the scalp yields 100mg of hair, which is the optimal quantity for both screening and validation. For EtG, additional tests, and/or assessments beyond 10 panels, it is advised to collect 150mg of the sample. We suggest confirming the sample weight using a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair can be gathered. When mentioning head hair, we specifically refer to scalp hair. Body hair encompasses all other types of hair (such as facial, axillary, etc.).

Process Overview

There are four primary phases in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of processing a sample into a laboratory’s system, ensuring the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not addressed by an electronic chain of custody system.

Screening consists of an initial swift assessment for drugs of abuse. Although Screening offers a cost-effective method to rule out drug use for most samples, a positive screen needs to be confirmed for legal admissibility. Samples that are presumptively positive during Screening require a subsequent confirmation.

Should a sample test presumptively positive at the Screening phase, further hair is taken from the original sample for Preparation for Extraction. This phase involves extracting drugs from hair at lower concentrations than other methods (e.g., urine or saliva), making hair drug screening the most complex methodology to execute.

Confirmation of any positive Screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as needed. The entire laboratory chain from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and with accreditation to ISO/IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
