When gathered near the scalp, hair can reflect up to a roughly 3-month period of substance exposure. Hair is straightforward to gather, somewhat resistant to tampering, and convenient to transport.

A 1.5-inch hair sample of approximately 200 strands (comparable to the size of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal sample amount for testing and verification. For EtG, additional analysis, and/or tests surpassing 10 panels, 150mg of the sample is advocated. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair isn't present, an equivalent quantity of body hair can be used. When mentioning head hair, we mean scalp hair solely. Body hair signifies all other types such as facial, axillary, etc.

Process Overview

The principal steps in the lab handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial treatment of a sample into a lab's framework. This includes ensuring the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing further data input not provided by an electronic chain of custody system.

Screening involves a preliminary swift check for abuse substances. While Screening is an efficient approach to eliminate drug use on most samples, a positive result must be verified to be legitimate in legal situations. Any samples that are believed to be positive in Screening demand additional confirmation.

If a sample shows presumptive positive at the Screening phase, further hair is extracted from the initial sample and prepared for Extraction. During this stage, drugs are extracted from hair at a notably lower concentration than in other procedures (such as urine or oral fluid), which makes hair drug testing the most challenging procedure to conduct.

Validation of any positive result from screening is performed through GC/MS, GC/MS/MS or LC/MS/MS. All suspected positive samples are cleaned before confirmation as necessary. The whole laboratory process from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair designation and certification to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
