A standard sample consists of a 1.5-inch section with around 200 hairs (comparable in thickness to a #2 pencil) from the scalp to yield 100mg, which is optimal for both screening and verification. For EtG, additional tests, or panels exceeding 10, a 150mg sample is suggested. Using a jeweler’s scale to weigh the sample is recommended. Should scalp hair be missing, an equivalent amount of body hair can be taken. "Head hair” denotes scalp hair, while “body hair” includes all other hair types (e.g., facial, axillary).

Process Overview

There are four essential stages in the laboratory processing for drug testing: Accessioning, Screening, Extraction, and Confirmation.

Accessioning starts with integrating a sample into the laboratory's system, which involves checking if the sample was correctly sealed and shipped, assigning a LAN (Laboratory Accessioning Number), and completing any necessary data entry absent from an electronic chain of custody system.

Screening contains an initial rapid evaluation for drug presence. Though Screening is an affordable method to exclude drug intake in most samples, results that show positive require further confirmation for legal acceptability. All samples suspected to be positive through Screening demand an additional confirmation stage.

If a sample shows a potential positive result during Screening, more strands are extracted from the original sample and prepared for Extraction. Here, drugs are isolated from hair at a significantly lower concentration than other methods (like urine or oral fluid), making hair drug screening one of the most challenging processes to execute.

Any positive results during screening are verified via GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are potentially positive undergo necessary washing before confirmation. The comprehensive lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
