For EtG, additional analyses, and/or tests exceeding 10 panels, a 150mg specimen is advised. It is recommended to weigh the sample with a jeweler’s scale. If scalp hair is absent, an equivalent amount of body hair can be gathered. When discussing head hair, it pertains solely to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The primary procedures in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial sample processing within a lab’s system. This entails checking the sample was sealed and sent correctly, creating a random LAN (Laboratory Accessioning Number), and completing extra data input absent from an electronic chain of custody system.

Screening constitutes an initial expedited assessment for drugs of abuse. Though Screening is a cost-effective method to eliminate the possibility of drug use in most samples, positive screenings must be confirmed for court admissibility. Any samples that are tentatively positive in Screening require further confirmation.

For any sample showing a tentative positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening exceptionally challenging to perform.

Confirmation of any positive screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleansed before confirmation as required. The entire lab procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the standard accreditation to ISO / IEC 17025.
