What it is, How it works

When gathered near the scalp, hair can offer up to an approximately 3-month window for detecting alcohol and other drugs. Hair collection is straightforward, somewhat resistant to tampering, and convenient to transport.

A 1.5-inch segment comprising around 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is deemed the optimal sample for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is advisable. We suggest using a jeweler’s scale for weighing the sample. In cases where scalp hair is not available, a comparable amount of body hair can be collected. When discussing head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning tasks involve the initial introduction of a sample into a lab’s system. This encompasses confirming the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input not included by an electronic chain of custody system.

Screening serves as an initial rapid check for drug abuse. While Screening is a cost-efficient means to exclude drug usage in most samples, a positive screen requires confirmation for it to be permissible in court. Any samples testing presumptively positive in Screening necessitate further confirmation.

If a sample is presumptively positive during Screening, more hair is extracted from the original sample and prepared for Extraction. This stage involves extracting drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), rendering hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Any samples testing presumptively positive undergo washing before confirmation as required. The entire lab procedure from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and meets the ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
