For EtG, additional tests, and/or assessments beyond 10 panels, 150mg of the specimen is advised. Using a jeweler’s scale to verify the specimen's weight is suggested. Should scalp hair be unavailable, an equivalent quantity of body hair is acceptable. When mentioning head hair, we exclusively mean scalp hair. Body hair pertains to all other types (facial, axillary, etc.).

Process Synopsis

The laboratory processing of a drug test result involves four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial incorporation of a sample into the lab's protocol. This procedure involves confirming the sample's correct sealing and shipping, designating a random LAN (Laboratory Accessioning Number), and fulfilling any supplemental data entry absent from an electronic chain of custody system.

Screening performs a preliminary assessment for drugs of misuse. Though Screening offers a cost-efficient means to exclude drug usage in most instances, a positive screen necessitates confirmation to be legally viable. Samples that preliminarily test positive in Screening must undergo further confirmation.

If a specimen is tentatively positive during the Screening stage, additional hair is extracted from the initial sample and prepped for Extraction. Drugs are drawn out of the hair at a significantly lesser concentration than alternative methods (e.g., urine or oral fluid), making hair testing the most challenging methodology to execute.

Confirmation of any initial positive results is achieved using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All presumptive positive specimens are cleansed before confirmation as required. The entirety of the laboratory process, from Accessioning through Confirmation, is subjected to review under the CAP (College of American Pathologists) Hair designation, as well as accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
