What it is, How it works

When collected near the scalp, hair can offer an approximate detection period of three months for alcoholic and other substances. Collecting hair is simple, relatively tamper-proof, and easy to transport.

A sample of 1.5 inches with around 200 hair strands (about the size of a #2 pencil) closest to the scalp yields 100mg of hair, which is the optimal amount for analysis. For tests such as EtG, additional tests, or those exceeding 10 panels, 150mg of hair is advisable. It is recommended to weigh the sample using a jeweler's scale. If there is no scalp hair, a similar quantity of body hair can be used. When mentioning head hair, it specifically refers to scalp hair only. Body hair includes all other types (facial, axillary, etc.).

Process Overview

Laboratory drug test processing consists of four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is processed into a laboratory's system. This involves verifying that the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and handling any supplemental data entry absent from an electronic chain of custody system.

Screening involves a preliminary check for drugs of abuse. Though a cost-efficient way to exclude drug use in the majority of samples, a positive screen necessitates confirmation to be valid in legal scenarios. Samples that yield a presumptive positive in Screening require further confirmation.

In case a sample tests presumptively positive during Screening, more hair is extracted from the original specimen and prepared for Extraction. Here, drugs are removed from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to conduct.

Confirmation of positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Samples that test presumptively positive undergo washing before confirmation as needed. The full laboratory process from Accessioning through Confirmation is conducted following the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
