A 1.5-inch segment of roughly 200 hair strands (comparable to the thickness of a #2 pencil) adjacent to the scalp will yield 100mg, the optimal amount for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is preferable. A jeweler’s scale is recommended for measuring the sample. In the absence of scalp hair, a comparable volume of body hair could be utilized. When mentioning head hair, we specifically refer to scalp hair, whereas body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test outcome in a lab comprises four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary incorporation of a specimen into a lab’s system. This stage confirms the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and finalizes any further data entry absent from an electronic chain of custody system.

Screening provides an initial overview for substances of abuse. Although Screening is an economical means to discount drug consumption for most specimens, a positive result requires confirmation for court admissibility. Any specimens deemed presumptively positive in the Screening stage necessitate follow-up confirmation.

In instances of presumptive positivity during the Screening phase, additional hair is taken from the original specimen for Extraction. In this phase, drugs are removed from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which contributes to the complexity of hair drug testing.

Any positive findings from Screening must be confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample undergoes washing prior to confirmation, as needed. From Accessioning through Confirmation, the complete lab protocol receives scrutiny under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards for accreditation.

The impact of hair treatments on the oxidation/decomposition of drugs in hair was also explored, as the long-term fate of drugs incorporated in the keratin matrix. Furthermore, the SoHT meeting in Verona featured research on the identification and quantification of alcohol abuse biomarkers, their stability, and their correlation with exposure and other biomarkers.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
