Process Overview

The lab processing of a drug test result includes four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the onset of sample integration into a laboratory’s system, which encompasses ensuring proper sealing and shipment of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing supplementary data entry absent from an electronic chain of custody system.

Screening offers an introductory rapid assessment for substances of abuse. While Screening effectively rules out drug usage for a majority of samples, any positive findings necessitate confirmation for validity in legal proceedings. Presumptively positive results require additional confirmation.

Should a sample be deemed presumptively positive in Screening, extra hair is extracted from the primary specimen and readied for Extraction. Confirmation of any positive Screening outcome is accomplished via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a washing process prior to confirmation if required. The entire lab workflow from Accessioning to Confirmation is scrutinized according to the CAP (College of American Pathologists) Hair designation and accreditation under ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
