What it is, How it works

Hair testing is increasingly recognized as a robust means for detecting drug and alcohol consumption. Hair captures a lasting record of alcohol and other substances by embedding biomarkers within the strands' fibers as they grow. When taken near the scalp, a hair sample can offer a detection span of up to roughly three months for alcohol and other drugs. Collecting hair is straightforward, not easily tampered with, and convenient to transport.

A 1.5-inch segment of around 200 strands of hair (approximately the thickness of a #2 pencil) nearest to the scalp is ideal, yielding 100mg for screening and confirmation tests. For EtG, additives, or tests exceeding 10 panels, a 150mg specimen is advisable. Using a jeweler's scale to weigh the specimen is recommended. Should scalp hair be unavailable, an equivalent volume of body hair can be used. References to head hair specifically mean scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory steps for processing drug test results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initial sample processing into the laboratory’s system, ensuring the sample's proper sealing and shipping, a random LAN (Laboratory Accessioning Number) assignment, and finalizing any extra data entry absent from an electronic chain of custody system.

Screening is an initial rapid check for drugs of abuse. While a cost-effective approach to eliminate drug use in most samples, a positive screen requires confirmation for court admissibility. Any samples with presumptive positives from Screening require secondary confirmation.

If a sample tests presumptive positive during Screening, additional hair from the initial sample is used for Extraction, where drugs are extracted from hair at lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening a challenging methodology.

Confirmation of any positive screening result is done via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation as necessary. From Accessioning to Confirmation, the entire lab process is assessed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
