What it is, How it works

Hair testing is regarded as an effective mechanism for identifying the use of drugs and alcohol. Hair offers a long-lasting record of alcohol and drug consumption by capturing biomarkers within the strands’ fibers as they grow. When taken near the scalp, hair can offer an approximate 3-month detection period for alcohol and drugs. Hair is straightforward to collect, challenging to tamper with, and convenient to ship.

Obtaining a 1.5-inch sample of about 200 hair strands (approximately the circumference of a #2 pencil) closest to the scalp will yield around 100mg of hair, which is optimal for screening and confirmation. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the sample is advised. It is suggested to use a jeweler’s scale to weigh the sample. If scalp hair is absent, an equivalent volume of body hair can be collected. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

Four primary stages are involved in the lab process of developing a drug test report: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into a laboratory’s database. Tasks include checking if the sample was sealed and shipped correctly, assigning a unique LAN (Laboratory Accessioning Number), and finishing any extra data entry not included in an electronic custody trail.

Screening is a preliminary, swift inspection for drug use. Although Screening is cost-efficient at excluding drug use in most samples, a positive screen must be validated to be admissible in court. Any samples that are presumptive positives in Screening must undergo a further confirmation.

Should a sample show a presumptive positive result during Screening, additional hair is extracted from the original sample for Extraction. In this phase, drugs are extracted from hair at a considerably lower concentration than in other methods (such as urine or oral fluid), which is why hair drug testing is the toughest methodology to execute.

Confirmation of any positive Screening outcome occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The entire lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO/IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
