What it is, How it works

Hair encases biomarkers within its strands across the growing period, offering a continuous record of alcohol and drug consumption. When obtained near the scalp, a hair sample can reveal drug or alcohol use for nearly three months. It is easy to gather, relatively hard to tamper with, and simple to transport.

A 1.5-inch section consisting of roughly 200 hair strands (similar to a #2 pencil in thickness) taken next to the scalp can provide 100mg, which is the optimal quantity for both preliminary screening and confirmation. For EtG evaluations, add-ons, and assessments exceeding 10 panels, a quantity of 150mg is advised. Utilizing a jeweler’s scale for specimen weight is suggested. If scalp hair isn't available, an equivalent volume of body hair can be used. Our mention of head hair specifically pertains to scalp hair. Body hair encompasses all other varieties (e.g., facial, axillary).

Process Overview

There are four primary phases in lab analysis of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the sample's preliminary processing into the lab’s records. This involves ensuring the sample’s seal and shipping are intact, assigning a random LAN (Laboratory Accessioning Number), and handling any extra data entry not included in an electronic custody chain.

Screening provides a preliminary check for drugs. Although Screening offers a cost-effective measure for ruling out drug use in most samples, confirmation is required for a positive screen to be court-admissible. Presumptive positive samples in Screening must undergo additional confirmation.

When a sample is presumptively positive in Screening, additional hair is taken from the original specimen for Extraction. This phase extracts drugs from hair at concentrations substantially lower than those seen in other methods (e.g., urine or oral fluid), establishing hair screening as a particularly complex method to execute.

GC/MS, GC/MS/MS, or LC/MS/MS technology is employed to verify any positive screen result. All presumptive positives are cleansed prior to confirmation as necessary. The uniformity of the laboratory procedures from Accessioning through to Confirmation is scrutinized in accordance with CAP (College of American Pathologists) Hair designation standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
