If scalp hair is unavailable, a comparable amount of body hair can be collected. By “head hair,” we mean only scalp hair. The term body hair relates to all other hair (facial, axillary, etc.).

Process Overview

Laboratory processing for a drug test outcome involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial handling of a sample within a laboratory's system. This entails verifying proper sealing and shipping of the sample, assigning a random Laboratory Accessioning Number (LAN), and completing extra data input if not available through an electronic chain of custody system.

Screening serves as an initial rapid evaluation for drugs of abuse. Although Screening is an economically efficient way to exclude drug use in most samples, a positive outcome needs confirmation to be presentable in court. Any samples deemed presumptively positive during Screening require a follow-up confirmation.

If a sample shows presumptive positivity in Screening, additional hair is extracted from the original specimen for further preparation in Extraction. During this phase, drug extraction from hair occurs at a lower concentration than other methods (such as urine or oral fluid), making hair drug screening a more challenging methodology.

Confirmation for any positive screening outcome is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, any samples marked presumptive positive are washed if necessary. The entire laboratory workflow from Accessioning to Confirmation undergoes review per the CAP (College of American Pathologists) Hair certification and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
